ELIGIBILITY
If you are a California resident with low or no income, you may be eligible for Medi-Cal. This includes the following:
- Low-income individuals
- Families with children
- Seniors
- Persons with disabilities
- Foster care
- Pregnant women
- Individuals with diseases such as tuberculosis, breast cancer, or HIV
Full Scope Medi-Cal eligibility:
- U.S. Citizens
- Children up to age 19, regardless of immigration status
- Adults aged 19-25, regardless of immigration status
- Adults aged 50 and older, regardless of immigration status
- All California residents, starting January 1st, 2024*
* Effective January 1st, 2024, adults age 26-49 will qualify for full scope Medi-Cal regardless of immigration status, if they meet all other eligibility requirements.
You can prove that you live in California with one of the following
- California driver's license (DL) or California identification (ID) card
- Check stub
- Lease, rent, or mortgage receipt
- Utility or telephone bill
- School, government, or any other document showing a California address
HOW TO APPLY
In Person: If you wish to apply in person, you may go to one of the following Alameda County Social Services offices. Our offices are open Monday through Friday between 8:30 AM and 5 PM.
By Mail: To apply for Medi-Cal, by mail, you can send your Medi-Cal application to an Alameda County Social Services Agency office.
To request a Medi-Cal application to mail in and instructions booklet, please call (510) 272-3663 or 1-800-698-1118 (toll free).
You may also click HERE for a Medi-Cal Printable Application. Fill-and-print PDF forms may be completed online and printed to hardcopy to be signed and mailed. Translated forms can be found HERE.
Remember, whether you take your application to the SSA or you mail it, you should not pay anyone to help you with the completion of this application.
