The Encore Medical Clinic (EMC) addresses the unique health needs of homeless children. It is a collaboration between the Center for the Vulnerable Child, providing outreach, comprehensive, culturally specific case management services while providing a medical home for homeless families. Besides medical services, assistance with insurance, housing, dental care, social services and legal services and legal referrals are offered. Our experienced case managers conduct outreach at community shelters and drop-in centers and accept referrals from other providers such as the ER. Our goal is to provide a consistent medical home for the children while assisting the family through their period of transition.
Comprehensive Healthcare for Homeless Children
Children living in transitional situations are less likely to have health insurance or a medical home, yet they're often the children who need it the most. Under the guidance of our Center for the Vulnerable Child, the Encore Medical Clinic seeks to alleviate some of the problems related to homelessness in children by providing medical case management.
Our federally-funded program is the only pediatric clinic providing outreach and case management services for homeless children in California.
Routine Care and Case Management Services
Encore Medical Clinic is staffed with three case managers who conduct community outreach to locate homeless families who lack health insurance or a pediatric medical home.
The Encore Medical Clinic provides 1) a continuous place for healthcare, 2) a personally assigned and consistent primary care physician, 3) ease with referrals, 4) comprehensive care coordination, and 5) family-centered care.
